Approval by the County Administrator for the Quality Improvement (QI) Award and Integrated Behavioral Health Services (IBHS) Y2 Grant Award for the Health Care for the Homeless (HCH) Program supplemental funding opportunities:

 

                     Pinellas County has been awarded $65,552.00 for its Quality Improvement (QI) efforts including its Patient Centered Medical Home (PCMH) recognition achieved this past year. The QI award is one-time funding for 12 months from the date of receipt, August 18, 2020 - August 17, 2021.

 

                     The IBHS award in the amount of $167,000.00 is for Year 2 supplemental grant funding to continue behavioral health integration in the Health Care for the Homeless (HCH) program.  The Integrated Behavioral Health Services supplemental award funds a full time Psychiatric provider co-located at the Bayside Clinic and Mobile Medical Unit.

 

                     The IBHS award provides the next 12 months of funding from September 1, 2020 - August 31, 2021.

 

                     There are no match requirements for either award.

Summary:

Each year, the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) awards health centers with one-time supplemental grant funding based on Quality Improvement (QI) measures reported from the Uniform Data System (UDS) performance reports. QI Funds must be used to support quality improvement activities which include, but are not limited to: developing and improving health center QI systems and infrastructure, including training staff, developing policies and procedures, enhancing health information technology, certified electronic health record, and data systems, data analysis; and/or developing and improving care delivery systems, including care coordination, case management, medication management, laboratory reporting and tracking, training and workflow redesign to support team-based care, clinical integration of behavioral health, oral health, HIV care, and other services; and/or patient engagement activities.

 

The Integrated Behavioral Health Services supplemental award funds a full-time Psychiatric Provider co-located at Bayside Health Clinic and Mobile Medical Van. This allows for real-time integration of primary and psychiatric care for homeless individuals in need of psychiatric services.

 

The HCH Co-Applicant Board will accept the awards on September 8, 2020.

 

Background/Explanation:

The U.S. Department of Health and Human Services announced over $117 million to 1,318 health centers in all U.S. states, territories and the District of Columbia as an investment in quality improvement, building upon their 2019 achievements.  Health centers will use these funds to expand current quality improvement systems and infrastructure and to improve primary care service delivery in the communities they serve.

Health centers are receiving these funds based upon high levels of performance in one or more of the following categories: Improving Quality of Care, Advancing Health Information Technology for Quality, Maintaining Patient-Centered Medical Home (PCMH) Recognition, Enhancing Value and Access to Care, and Reducing Health Disparities.

 

The Health Care for the Homeless program served a total of 2,951 unique patients in Calendar Year 2019, 498 (16.8%) of whom received a total of 2,826 mental health service visits.

 

 

Fiscal Impact:

Total grant award is $232,552.00 in federal funds for the Health Care for the Homeless Program and no match is required. This award is not included in the FY21 Tentative Budget. A resolution supplementing the FY21 budget recognizing these additional funds may be needed.

 

Delegated Authority:

Authority for the County Administrator to approve this Notice of Grant Award is granted under (Code Section 2-62 (a)(1).
