Approve the Interlocal Agreement with the Florida Department of Health in Pinellas County (DOH) for the Pinellas County Health Program (PCHP) and the Health Care for the Homeless (HCH) program.

Summary:

Funding for this Agreement enables DOH to provide healthcare services for eligible, enrolled PCHP/HCH clients at six (6) locations throughout the County, as well as the Mobile Medical Unit (MMU) and the Bayside Clinic.  Additionally, DOH manages and provides, through sub-contracted agreements, specialty care, behavioral health services, prescription assistance and dental services.  DOH works with Human Services (HS) to ensure that the services provided are coordinated and efficient.

 

Background Information:

Pinellas County Human Services (HS) has partnered with the DOH to provide prevention-focused healthcare to eligible Pinellas County residents.  DOH has a proven system of providing high quality, effective and cost-efficient services to the residents of Pinellas County. 

From October 2017 through May 2018, DOH provided 9,268 medical encounters and 3,782 dental encounters to PCHP clients, and 4,069 medical encounters and 1,682 dental encounters to HCH clients.

HS and DOH continue to collaborate around expanding access to critical services.  As part of this collaboration, in July 2016, the HCH program achieved designation as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA).  Ongoing efforts are working to improve discharge planning from hospitals for continuity of care and helping to connect clients to behavioral health services and medication assisted treatment for mental health, substance abuse, and co-occurring disorders. 

 

Fiscal Impact:

Program funding is not to exceed $11,662,170.00 per fiscal year (FY) derived by a combination of General Fund appropriation and grant funding through the Health Resources and Services Administration (HRSA).  Funding in the amount of $11,662,170.00 has been included in Human Services’ FY19 tentative budget request. Grant funds total approximately $959,819.00 for both the current year (ending February 28, 2019) and the next year (starting March 1, 2019).
