Approve and execute the Second and Final Option of Renewal and Amendment 3 to the Community Home Care Medical Services Agreement with BayCare Home Care, Inc., (BCHC) for services related to clients of the Pinellas County Health Program (PCHP) and Health Care for the Homeless (HCH) program.

Summary:

This agreement provides home care services for authorized PCHP/HCH clients to aid in stabilizing health and recovery.  Services include durable medical equipment, home health services, oxygen, outpatient rehabilitation services, wound care, infusion therapy, and orthotics and prosthetics at agreed upon discounted rates.  This renewal includes an amendment to cap service delivery at $550,000.00 for BCHC. 

 

Background Information:

BCHC provides home health care services for residents enrolled in PCHP/HCH in cooperation with partnering hospitals and providers.  PCHP/HCH clients consist of uninsured adults between the ages 18-64 who are in need of healthcare, but meet certain income level criteria and cannot afford care on their own.  The goal of the PCHP program is to provide primary medical care services through a patient-centered medical home and to divert emergency room visits.  BCHC provides broad geographical coverage throughout the County. 

This agreement includes an amendment to cap service delivery for BCHC at $550,000.00.  Previous agreement terms required BCHC to continue to deliver services to clients once the contracted funding was expended to ensure continuity of care.  In recent years, BCHC has seen an increase in the charitable contribution to the contract and requires a cap on the service delivery set to the actual contracted amount.  To ensure service needs are met within the allocated budget, BCHC, the Florida Department of Health in Pinellas County (DOH) and Human Services have established a service review group which meets bimonthly.  Along with the review group, DOH performs a weekly review on enrolled clients to manage service levels and a standard policy is under development to establish service thresholds for clients.  These efforts will continue in the new fiscal year. 

From October 2016, through June 2017, BCHC provided 64,541 services to 810 PCHP clients.  The average cost per client was $575.21.

 

Fiscal Impact:

Funding in an amount not to exceed $550,000.00 has been included in the Human Services’ proposed General Fund appropriation for Fiscal Year 2018 (FY18).
