Recommended Action

Approve the First Option of Renewal and Amendment No. 2 to the Community Home Care Medical Services Agreement with BayCare Home Care, Inc. for services related to clients of the Pinellas County Health Program (PCHP).

Body

Strategic Plan:

Ensure Public Health, Safety, and Welfare
2.1 Provide planning, coordination, prevention, and protective services to ensure a safe and secure community
2.2 Be a facilitator, convener, and purchaser of services for those in need

Summary:

Funding in the amount of $550,000.00 provides durable medical equipment, home health services, oxygen, outpatient rehabilitation services, wound care, infusion therapy and orthotics and prosthetics at agreed upon discounted rates for residents enrolled in the PCHP.

Specialty home health medical services are limited to a total of thirty (30) visits per client per year.  This action renews the agreement for one year and amends the internal contact for the agreement.

Background Information:

PCHP clients consist of uninsured adults between the ages 18 and 64 who are in need of healthcare, but meet certain income level criteria and cannot afford care on their own.  The goal of the PCHP is to provide primary care medical services and to divert emergency room visits.  BayCare Home Care provides home delivery of medical equipment with broad geographical coverage throughout the County.

From Jan. 2015 through Mar. 2016, BayCare Home Care provided 1,638 services for clients of PCHP.  These services included 301 skilled nursing visits, 973 requests for durable medical equipment or medical supplies, and 364 other services ranging from therapy to intravenous (IV) support.

Fiscal Impact:

Funding to support this contract in the amount of $550,000.00 is provided through the Human Services anticipated annual appropriation for Fiscal Year 2017.
