Summary:
The FY 2016 DSHII one-time supplemental funding opportunity is for existing health centers program award recipients to provide support for strategic investments in health information technology (health IT) for enhancements to accelerate health centers’ transition to value-based models of care, to improve efforts to share and use information to support better decisions, and increase engagement in delivery system transformation. Pinellas County is seeking to use the investment to implement new clinical and administrative workflows, develop new health center specific reports, improve clinical decision making, and more effectively manage patients within the health center and across the health care system, ultimately improving patient and population health outcomes.
The Mobile Medical Unit Advisory Council, co-applicant governing board, approved the submission of the grant application at the July 12, 2016 meeting.
Background/Explanation:
On March 1, 2016, Pinellas County was awarded a new three-year project period as a Health Care for the Homeless health center grantee by HRSA to provide primary and preventive care to homeless individuals in the County. In 2015, the health center served 2,239 unduplicated homeless individuals.
Fiscal Impact:
One-time supplemental federal funding in the amount of $42,239.00 to the Health Care for the Homeless grant (H80CS00024). No match is required.

Authority for the Department Director to sign this grant application is granted by the memo dated March 28, 2016 titled “Delegation of Authority from the County Administrator and Improved Grant Procedures.”
