Summary:
The FY16 DSHII one-time supplemental funding award is for existing health centers program recipients to provide support for strategic investments in health information technology (health IT) for enhancements to accelerate health centers’ transition to value-based models of care, to improve efforts to share and use information to support better decisions, and increase engagement in delivery system transformation. Pinellas County is seeking to use the investment to implement new clinical and administrative workflows, develop new health center specific reports, improve clinical decision making, and more effectively manage patients within the health center and across the health care system, ultimately improving patient and population health outcomes.
The Mobile Medical Unit Advisory Council, co-applicant governing board, approved the acceptance of the grant award at the October 4, 2016 meeting.
Background/Explanation:
On March 1, 2016, Pinellas County was awarded a new three-year project period as a Health Care for the Homeless health center grantee by HRSA to provide primary and preventive care to homeless individuals in the County. In 2015, the health center served 2,239 unduplicated homeless individuals.
This new award allocation will provide $42,239.00 to enhance service coordination for continuity of care. Anticipated outcomes include 1) improved workflow through additional training days, two (2) added printers on Mobile Medical Unit, and added licensing for the system and 2) improved referral connections by implementing continuity of care process with behavioral health providers across electronic health records.
Fiscal Impact:
One-time supplemental federal funding in the amount of $42,239.00 to the Health Care for the Homeless grant (H80CS00024). No match is required.
Delegated Authority:
Authority for the County Administrator to accept this grant award is in accordance with Pinellas County Administrative Code (2-62 (a) (1)).
