Approve the grant agreement for Community & Primary Care Services from the Florida Agency for Health Care Administration (AHCA).
The Community & Primary Care Services grant agreement (#GFA049) is in the amount of $485,419.12 and shall begin upon execution by both Parties through June 30, 2016.
The Grant Application was approved as a delegated item on Sept. 30, 2015. The agency awarded the County the grant Oct. 23, 2015, and sent the agreement to the County for approval on Feb. 11, 2016.

Summary:
The County, through this Community & Primary Care Services grant will increase access to behavioral health services and prevent unnecessary emergency room visits and inpatient hospitalizations. The AHCA grant will provide funding for on-site behavioral health services that are fully integrated with the primary care services offered by the Health Care for the Homeless (HCH) program. Funding is available to serve approximately fifty (50), uninsured, low-income, homeless clients per month.
The County will partner with the Florida Department of Health in Pinellas County, BayCare Behavioral Health and Directions for Living to provide services. BayCare will provide staff on-site at the Bayside Health Clinic and co-located with the Mobile Medical Unit (MMU) in community locations. Directions will provide additional therapists to serve longer-term needs; and the Florida Department of Health will provide an additional nurse to provide clinical care coverage in the evening and on weekends at Safe Harbor.
Background Information:
The County Administrator approved through delegated authority the department’s grant application to AHCA for the Community & Primary Care Services grant on September 30, 2015. The agency awarded the County the grant on October 23, 2015 and sent the grant agreement to the County on February 11, 2016 for approval. The agreement term is from the date of execution by both parties through June 30, 2016.
This program is designed to meet an immediate need for behavioral health/substance abuse stabilization and intervention through increased access to behavioral health and primary care services. The program will provide disease management, in the form of management of mental illness and substance abuse. In addition, this program will improve patient compliance and coordination with primary medical care, referrals, and connecting patients to other community resources.
Fiscal Impact:
The grant agreement is in the amount of $485,419.12.
Grant funding has been approved for carryover in the Fiscal Year 2017 (FY17) AHCA budget. A renewal of the agreement will be processed for FY17.
