Approval of Change Order No. 1 to the contract with Pepper Contracting Services, Inc (Pepper). for the Forest Lakes Boulevard Phase II project (west of Pine Avenue to west of Race Track Road).

 

                     This project consists of adding capacity to this corridor with the widening/reconstruction of Forest Lakes Boulevard, from west of Pine Avenue to Racetrack Road, from a two-lane divided suburban roadway to a divided four-lane suburban typical section.

                     Change Order No. 1 extends the agreement term by seventy consecutive calendar days and increases the contract by $4,105,371.00.

                     Change Order No. 1 is necessary due to highly variable ground conditions discovered during construction. Ground conditions resulted in pavement failure of recently completed segments in the eastbound lanes. Further evaluation is necessary to determine if the required rework may be attributable to the Engineer of Record; however, the work must continue.

                     The Board of County Commissioners awarded this contract on October 9, 2018 in the amount of $12,025,336.10.

 

Contract No. 178-0423-CP(PLU); PID No. 002110A in the amount of $4,105,371.00, for a revised total of $16,130,707.10, to be completed by September 28, 2021. Authorize the Chairman to sign and the Clerk of the Circuit Court to attest following signature approval by Pepper.

Summary:

Summary

Change Order No. 1 increases the contract by $4,105.371.00 due to highly variable ground conditions discovered during construction. Ground conditions resulted in pavement failure of recently completed segments in the eastbound lanes. The observed conditions of failure Included cracking, yielding when construction equipment passed over, and settlement in some locations.

The remediation plan consists of installing a layer of geosynthetic reinforcement (geogrid) beneath the subgrade layer and utilizing 12" of recycled crushed concrete as subgrade In lieu of the stabilization as called for in the bid plans. The remainder of the road pavement layers are to be constructed per the bid plans. The change order is required to purchase and install the geogrid and associated material for the failed sections and remainder of the project.  Further evaluation will be required to determine if the required rework may be attributable to the Engineer of Record. The change order also adds seventy (70) consecutive calendar days through September 28, 2021.

Background Information:

The purpose of this project is to add capacity to this corridor with the widening/reconstruction of Forest Lakes Boulevard, from west of Pine Avenue to Racetrack Road, from a two-lane divided suburban roadway to a divided four-lane suburban typical section. Using a portfolio approach, the construction activities include roadside drainage, new drainage pipes, underdrain, signing and pavement markings, signals, intelligent transportation systems, lighting, paved bicycle lanes, sidewalks, and pedestrian features. All work is within the existing right-of-way with mast arms and
pedestrian signals to be constructed at Pine Avenue and Brooker Creek Boulevard. The project also includes a Joint Project Agreement with the City of Oldsmar for water, sewer, reclaimed water, and fiber optic communication line relocations.

 

Fiscal Impact:

Total expenditure not to exceed:                            $12,025,336.10

Increase due to Change Order No. 1:                    $  4,105,371.00

Revised expenditure not to exceed:                       $16,130,707.10

 

Funding is derived from the Local Option Infrastructure Sales Surtax (Penny for Pinellas) in the amount of $9,610,860.10, partial grant funding from the Florida Department of Transportation (FDOT) through a County Incentive Grant Program agreement in the amount of $1,953,091.00, and funding from the City of Oldsmar in the amount of $461,385.00 for their portion of utility relocation work.
