Approval of the Second Amendment to the Agreement with Operation PAR, Inc. (PAR) to incorporate additional funding from the 2018 Expanding Access to Quality Substance Use Disorder and Mental Health Services Grant Award (SUD-MH) and the Medicaid Low Income Pool program (LIP) to the Health Care for the Homeless (HCH) Medication Assisted Treatment (MAT) program.

 

                     SUD-MH will provide an additional full-time counselor and care coordinator as an intervention team for HCH client referrals from St. Anthony’s Hospital and existing high-need HCH clients to encourage connections to treatment and engagement in MAT and co-occurring treatment services. 

                     Three additional mental health staff (a therapist, a nurse, and a team coordinator) will be added to the intervention team using LIP funding in partnership with St. Anthony’s Hospital and BayCare to provide ongoing services for client stabilization.

                     This program will provide an additional 28 clients annually with MAT services for substance use disorders and will serve approximately 65 high need clients with mental health interventions. 

                     Total contract amount for approval is $585,750.00 ($315,750.00 in SUD-MH funding and $270,000.00 in General Fund health program funding allocated through LIP).

Summary:

SUD-MH and LIP will provide additional funding to HCH for the period of September 1, 2018 through August 31, 2019.  The additional funding will expand client interventions, improve outreach, and allow for service coordination between PAR and BayCare. 

SUD-MH will provide an additional fulltime counselor and care coordinator, as an intervention team, for HCH client referrals from St. Anthony’s Hospital and existing high need HCH clients to encourage connections to treatment and engagement in MAT and co-occurring treatment services.  Additionally, three mental health staff (a therapist, a nurse, and a team coordinator) will be added to the intervention team using LIP funding in partnership with St. Anthony’s Hospital and BayCare Behavioral Health, Inc. (BayCare) to provide ongoing services for client stabilization.

This program will provide an additional 28 clients annually with MAT services for substance use disorders and will serve approximately 65 high need clients with mental health interventions.  In addition, the program will include approximately 268 bed days of residential substance abuse treatment to assist with stabilizing and engaging clients. 

 

Background Information:

In calendar year 2017, St. Anthony’s Hospital had 461 encounters with HCH clients where behavioral health was the principal diagnosis.  In addition, the Pinellas County Health Program has seen 117 clients with three or more Baker Acts over the period from January 15, 2016 through September 30, 2018.  Without dedicated interventions available for these high-need clients, these individuals often become revolving users of deeper end services. 

This program establishes a co-occurring intervention team as a collaborative approach across PAR and Baycare to stabilize and provide ongoing services for HCH clients.  The program will build off an existing partnership between PAR and St. Anthony’s Hospital that seeks to connect emergency room diversion to substance abuse treatment.

 

Fiscal Impact:

Funding in an amount not to exceed $315,750.00 is provided by Grant H80CS00024-17-02 from the Health Resources Services Administration for the period of September 1, 2018 through August 31, 2019 (of the $315,750.00, $140,750.00 is recurring annual funding and $175,000.00 is one-time funding).  No match is required.

 

Funding in an amount not to exceed $270,000.00 has been included in the Human Services Fiscal Year 2019 (FY19) Adopted Budget in the General Fund via LIP funding.

 

This amendment increases the total agreement amount by $585,750.00.  A budget resolution may be submitted in FY19 for $315,750.00 to recognize the unanticipated revenue and expenditures associated with the SUD-MH award.
