Approve and execute the Second and Final Option of Renewal and Amendment 3 in the Interlocal Agreement with the Florida Department of Health in Pinellas County (DOH) for the Pinellas County Health Program (PCHP) and the Health Care for the Homeless (HCH) Program.

Summary:

This agreement allows DOH to continue to provide primary care, preventive health, behavioral health, dental services, prescription assistance and specialty care services, for uninsured adults ages 18-64 who are in need of health care, but meet certain income level criteria and cannot afford care on their own.  DOH operates six medical homes, the Bayside Clinic, and the Mobile Medical Unit to provide over 9,947 PCHP and HCH clients with access to quality health care, improved health outcomes, and reduced health disparities with quality assurance.

The amendment provides a slight increase of $121,382.00, or approximately 1%, over Fiscal Year 2017 (FY17) funding levels due to an anticipated pay adjustment for state employees.

 

Background Information:

DOH has been a medical primary care provider for the PCHP since its inception in 2008.  In the first three quarters of FY17, DOH provided primary care services to 7,291 unique clients and dental services to 2,233 unique clients. In 2016, DOH received the Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance.  The PCMH is a promising model for transforming an organization and the delivery of high-quality, accessible, efficient health care.  A goal of the PCHP/HCH program is to prevent, diagnose, and/or treat medical and behavioral health concerns before they become an urgent matter.

As part of Human Services’ ongoing partnership with DOH, quarterly hospital coordination meetings continue to be held to work on discharge planning for clients.  In addition, coordination discussions continue with the Pinellas County Jail, Personal Enrichment through Mental Health Services (PEMHS), and Operation PAR, Inc. to explore opportunities to stabilize and reengage clients. 

 

Fiscal Impact:

Funding in an amount not to exceed $11,662,170.00 has been included in the Human Services’ proposed General Fund budget for FY18.
