Approve the amendment to the agreement with the Florida Department of Health in Pinellas County (DOH) to allocate new grant funding for County residents who are indigent, homeless or at risk of homelessness.
Request approval for the Chairman sign and the Clerk to attest.
The first amendment allocates an additional $746,305.00 to the approved contract amount through September 30, 2016 of $10,794,483.00 for primary care, specialty care, behavioral health care, laboratory services and dental care. The total revised FY16 contract amount is up to $11,540,788.00. This agreement may be renewed for up to two additional one year terms.

Summary:
The first amendment to the agreement with DOH allocates $746,305.00 in new grant funding from the U.S. Health Resources and Services Administration and, contingent upon receipt of funding, includes $294,219.00 from the State of Florida Agency for Health Care Administration (AHCA) for expansion of services, quality improvement and emergency room diversion in the HCH program (Mobile Medical Unit and Safe Harbor Homeless Shelter). The amendment also reallocates $25,000.00 for lab costs that will help to continue a pilot denture program in partnership with DOH.
Background Information:
The County applied for and/or received additional funding and funding adjustments for the HCH program contracted to the Department of Health. These include awards to expand service hours to evenings/weekends and to perform quality improvement, and a base funding adjustment.
Additionally, the County was notified of an award from AHCA for expanded behavioral health services to avoid unnecessary emergency room or hospital admissions. Funds for this state grant are added to the amendment contingent upon receipt of a contract from AHCA.
Human Services is also seeking to continue a pilot denture program that has been successful in providing seventy (70) individuals with dentures through DOH. By reallocating $25,000.00 to lab costs, it is anticipated that the County will be able to provide dentures to up to fifteen (15) clients per month or one hundred eighty (180) clients during FY16.
After the grant increases, the total grant funding from all sources for the HCH program is $1,950,844.00. The total amount of the Pinellas County Health Program Interlocal Agreement with the first amendment is up to $11,540,788.00.
From January 1, 2015 through September 30, 2015, the HCH program served 1,794 unduplicated clients.
Fiscal Impact:
Amendment 1 represents grant funding received and accepted by the County and minor adjustments to the budget to accommodate a pilot dental program to provide dentures to 180 people in FY16:
• Notice of Award (8-18-15) of $148,790.00 for one-time grant supplement from HRSA recognizes demonstrated improvement in access, quality of care and value of care in 2014.
• Notice of Award (9-10-15) of $248,872.00 for Expanded Services from HRSA supports expansion of primary care services to include evening and weekend hours at the Safe Harbor Shelter.
• Notice of Award (9-21-15) of $54,424.00 increases the ongoing Base Funding level from HRSA.
• Preliminary Notice of Award (10-20-15) of $487,002.00 for Community & Primary Care Services Grant (RFA 001-15/16) from the Florida Agency for Health Care Administration. Of this total award, Amendment 1 allocates $294,219.00 to the Department of Health and its subcontractor, Directions for Mental Health, contingent upon receipt of funding from AHCA. The remainder ($192,782.00) will be contracted to another vendor.
• A budget resolution will be submitted for the Board of County Commissioners to recognize the new grant receipts and to increase the FY16 Human Services Department expense budget for this Amendment 1.
