Summary:
The Amendment submitted by AHCA changes three areas of the grant agreement including 1) Project Period - the agreement shall begin retroactive to April 13, 2016 and end on June 30, 2017, 2) Background Screening requirements, and 3) Performance Standards and Liquidated Damages.
Background/Explanation:
The County Administrator approved through delegated authority the department’s grant application to AHCA for the community and primary care services grant on September 30, 2015. The agency awarded the County the grant on October 23, 2015 and sent the grant agreement to the County on February 11, 2016 for approval. The agreement was approved by the Board on March 29, 2016 and fully executed by AHCA on April 13, 2016.
The County, through this community & primary care services grant will increase access to behavioral health services and prevent unnecessary emergency room visits and inpatient hospitalizations. The AHCA grant will provide funding for on-site behavioral health services that are fully integrated with the primary care services offered by the Health Care for the Homeless (HCH) program. Funding is available to serve approximately fifty (50), uninsured, low-income, homeless clients per month.
The County will partner with the Florida Department of Health in Pinellas County, BayCare Behavioral Health and Directions for Living to provide services. BayCare will provide staff on-site at the Bayside Health Clinic and co-located with the Mobile Medical Unit in community locations. Directions will provide additional therapists to serve longer-term needs; and the Florida Department of Health will provide an additional nurse to provide clinical care coverage in the evening and on weekends at Safe Harbor.
