Approval by the County Administrator of the grant award for the U.S. Department of Health and Human Services for the Fiscal Year (FY) 2019 Health Center Quality Improvement one-time grant supplement for the Health Care for the Homeless program.

                     The FY19 Health Center Quality Improvement (QI) award provides one-time funding to support quality improvement activities which include, but are not limited to:  infrastructure, data systems, staff training, service delivery and partnerships, care coordination, workflow, service integration, and patient engagement. 

                     This QI award provides one-time grant funding $56,839.00 for use during the period of August 12, 2019, through July 31, 2020.

                     There is no match or cost sharing requirements.

                     The Health Care for the Homeless (HCH) program served 2,839 unique individuals in 2018 with preventive primary medical care services. Body

Summary:

Each year, the U.S. Department of Health and Human Services awards health centers with one-time supplemental grant funding based on quality improvement (QI) measures reported from the 2017 and 2018 Uniform Data System (UDS) performance reports.

Pinellas County Board of County Commissioners was awarded $56,839.00 in FY 2019 for its quality improvement efforts including its Patient Centered Medical Home (PCMH) Recognition. The award provides one-time funding for use during the period of August 12, 2019, through July 31, 2020. Funds must be used to support quality improvement activities which include, but are not limited to: developing and improving health center systems and infrastructure, including training staff; data analysis, and/or implementing targeted QI activities; developing and improving care delivery systems, supporting care coordination, case management, and medication management; developing and implementing contracts and formal agreements with other providers; laboratory reporting and tracking; training and workflow redesign to support team-based care; clinical integration of behavioral health, oral health, HIV care and other services; and/or patient engagement activities.

The County will be working closely with the Florida Department of Health in Pinellas to maintain our PCMH recognition and provide additional training to clinical staff. The HCH Co-Applicant Board is expected to accept the award on September 10, 2019.

 

Background/Explanation:

The U.S. Department of Health and Human Services announced nearly $107 million in Quality Improvement Awards (QIA) to 1,273 health centers across all U.S. states, territories and the District of Columbia as an investment in quality improvement. Health centers will use these funds to expand current quality improvement systems and infrastructure and to improve primary care service delivery in the communities they serve.

Quality Improvement Awards (QIAs) recognize health center achievements in: improving cost-efficient care delivery, while also increasing quality of care; reducing health disparities; increasing both the number of patients served and patients’ ability to access comprehensive services; advancing the use of health information technology; and delivering patient centered care.

The Health Care for the Homeless program served 2,839 unique individuals in 2018 with preventive primary medical care services.

 

Fiscal Impact:

The grant award totals $56,839.00. There is no match required.  This supplemental funding was not included in the FY20 Tentative Budget.  A Budget Resolution may be necessary in FY20 as a result of this award.  

Delegated Authority:

Authority for the County Administrator to accept this grant award is granted under Code Section 2-62 (a)(1).
