Approval and execution by the County Administrator of the Tangible Personal Property Report, as required by the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA), for closeout of the FY2021 American Rescue Plan Act Funding for Health Centers grant that funded the minor alteration & renovation of the Bayside Health Clinic.
● The FY2021 HRSA American Rescue Plan Act Funding for Health Centers grant was awarded in the amount of $1,567,250.00 for the period of performance starting April 1, 2021 through June 30, 2024.
● $352,500.00 of this grant award supported the minor alteration/renovation (A/R) and equipment costs for Phase II of the Bayside Health Clinic expansion project.
● Minor A/R included the removal of walls within the existing exam room hall, dental lab, and storage rooms to configure more-usable workspace for clinic staff. Equipment purchased included desks for consultation rooms and two cubicle workstation pods for the new open work area.
● Approval of this report has no fiscal impact on Pinellas County.

Summary:
Grantees are required submit the SF-428 Tangible Personal Property Report to document federally-acquired equipment with an acquisition cost of $5,000.00 or more for which HRSA has reserved the right to transfer title, and residual unused supplies with total aggregate fair market value exceeding $5,000.00. Records for equipment acquired with federal funds shall be retained for three years after final disposal.
Background/Explanation:
The Bayside Expansion Project was broken down into three phases of construction. Phases I & III expanded the capital footprint of the clinic by adding 2,400 square feet of medical, dental, behavioral health and lobby space. Phase II consisted of the minor alteration/renovation of existing clinic areas with the removal of walls within the exam rooms, hall, dental lab, and storage rooms to configure more-usable workspace for clinic staff, as well as furniture to outfit the new working spaces.
Phase II of the Bayside Expansion Project was completed on June 30, 2024, with final grant closeout documents due to HRSA by October 30, 2024.
A Certificate of Completion for all three phases was obtained on August 5, 2024.
