Approval of grant awards with the United States (U.S.) Department of Health and Human Services (HHS), Health Resources Services Administration (HRSA) for the Health Care for the Homeless Program.

Grant awards in the amount of $148,790.00, $248,872.00, and $54,424.00 for a total of $452,086.00.

Summary:

On August 18, 2015, the County received a Notice of Award from HRSA awarding the County $148,790.00, a one-time grant supplement from the Health Center Quality Improvement (QI) Fund, to recognize health centers that demonstrated improvements in access to care, quality of care, and/or value of care through data provided in the Calendar Year 2014 Uniform Data System (UDS). 

On September 10, 2015, the County received a Notice of Award from HRSA awarding the County $248,872.00 in supplemental funding for Expanded Services (ES) activities for the period September 1, 2015 through August 31, 2016 to support the expansion of comprehensive primary care services to underserved populations.  With this funding award, the County plans to expand hours of primary care services at the Safe Harbor shelter (and Bayside Clinic when open in the Spring of 2016) until 8 p.m., Monday through Thursday, and from 8 a.m. - 12 p.m. on Saturdays.

On September 21, 2015, the County received a Notice of Award from HRSA awarding the County $54,424.00 for an increase to the County’s annual ongoing base funding in accordance with statutory requirements and, as applicable, new/ and/or continued patient centered medical home recognition for one or more health center sites.

These notices (H80CS00024-14-11, H80CS00024-14-12, and H80CS00024-14-13) bring the total grant funding to $1,463,842.00 for the budget period ending February 29, 2016.  

 

Background/Explanation:

On November 18, 2014, the Board approved the grant award in the amount of $581,256 from HRSA for the Healthcare for the Homeless Program and authorized the County Administrator to approve additional HRSA grant funding for the Mobile Medical Unit that may occur during the grant year (November 1, 2014 through October 31, 2015). Additional awards approved by the County Administrator prior to this action brought the accepted funding total to $1,011,756.00.

HRSA, through the Notice of Award dated May 20, 2015, notified the County of a change in the budget period end date to February 29, 2016.

From January 1, 2015 through September 30, 2015, the HCH program has served 1,794 unduplicated patients.  The patient target for the calendar year 2015 is 2,390 unduplicated patients.

 

Fiscal Impact:

No match is required for this $452,086.00 in grant awards for the Healthcare for the Homeless Program.   Human Services does provide supplemental funding. This does not affect the FY2015 budget.  Human Services will submit a request to amend the FY2016 budget for new revenue and expenditures associated with the supplemental and expansion grant funds. 

 

Delegated Authority:

Authority for the County Administrator to accept this Notice of Award was granted under item #15 on November 18, 2014 by the Board of County Commissioners.
