Summary:
These Amendments adjust the current level of funding for two hospital partners based upon utilization rates of PCHP/HCH clients during Fiscal Year 2017 (FY17). The new funding amounts of $135,000.00 for FHNP and $801,000.00 for Bayfront provide partial reimbursement for ambulatory and inpatient hospital care related services for authorized clients who are actively enrolled in the PCHP/HCH programs.
Background/Explanation:
The PCHP/HCH programs provide primary care, behavioral health care, specialty care services, and some hospital procedures for uninsured adults ages 18-64 who are in need of healthcare but meet certain income level criteria and cannot afford care on their own. The goal of PCHP/HCH is to prevent, diagnose, and/or treat medical and behavioral health concerns before they become an urgent matter. Under this agreement, FHNP and Bayfront will continue to coordinate with the PCHP/HCH programs to provide ambulatory surgical center outpatient procedures and services, inpatient procedures and services, and patient rehabilitative services.
The total funding available across hospital agreements is $3,000,000.00. Based on reported service level data received, the FY19 breakdown is as follows: BayCare receiving 68.8%, Bayfront receiving 26.7%, and Florida Hospital North Pinellas receiving 4.5% of the funding allocation.
Human Services (HS) continues to partner with participating hospitals to implement effective policies and procedures for clients. Quarterly hospital coordination meetings are held to identify gaps and needs, and to explore opportunities for additional cost savings and care coordination. From these meetings, partners have worked to enhance eligibility screening processes with the hospitals, improve discharge planning coordination with the Florida Department of Health in Pinellas (DOH), and identify high use emergency room clients to look for trends and treatment needs. HS will continue to facilitate quarterly meetings with our partners in FY19 to further the progress on these efforts.
Fiscal Impact:
Program funding in an amount not to exceed $135,000.00 for FHNP and $801,000.00 for Bayfront for FY19 is included in Human Services’ FY19 Adopted Budget.

Authority for the County Administrator to sign these Amendments is granted under Code Section 2-62 (a)(1).
