Approval and execution by the County Administrator of the Health Care Clinic Establishment Permit Change of Designated Qualifying Practitioner (QP) Form for the Bayside Health Clinic of the Health Care for the Homeless Program.
• This permit is required by the Florida Department of Health to continue providing prescription drugs for its clients in compliance with the provisions of the Florida Drug and Cosmetic Act (Chapter 499, F.S. and Rule 61N-1).
• The Pinellas County Medical Director Mahima Pandey, M.D. will serve as the required Designated Qualifying Provider responsible for ensuring all related regulatory requirements at the clinic. The Qualifying Practitioner Form is being resubmitted for signature due to an administrative error by DBPR where they missed an interim change previously submitted that changed the QP from Dr. Chitra Ravindra to Dr. Ulyee Choe. So, this new form must indicate a change from Dr. Ravindra to Dr. Pandey compared to the previous submittal of Dr. Choe to Dr. Pandey.
• As the owner of the Bayside Health Clinic, Pinellas County will need to sign the attached Change in Qualifying Practitioner form and Health Care Clinic Establishment Signature Page as Owner or Officer.

Summary
The Florida Department of Health requires the submission of the Health Care Establishment Permit to comply with the provisions of the Florida Drug and Cosmetic Act (Chapter 499, F.S. and Rule 61N-1). Because the Bayside Health Clinic provides health care services on-site and needs to purchase prescription drugs for its clients, it needs a Health Care Clinic Establishment permit issued by the Florida Department of Business & Professional Regulation (DBPR) Division of Drugs, Devices, and Cosmetics (DDC). This license is intended to safeguard the health, safety, and welfare of the citizens of the state of Florida from injury due to the use of adulterated, contaminated, misbranded drugs, drug ingredients and cosmetics by administering the provisions of the Florida Drug and Cosmetic Act (Chapter 499, F.S. and Rule 61N-1).
